Some additional thoughts on your questions:
1. Joseph Smith "not worthy" might not have always been a correct explanation; the "time not being right" would be a better explanation for Joseph Smith not receiving the plates in most of the instances I believe.
2. Joseph Smith received many revelations and ministration of angels between the first visit by Moroni, and receiving the plates, the details of which are not recorded in his history. It is misleading to portray those highlighted events as his only communication with God during that period.
3. The document known as the "Anthon Transcript" is of a dubious authenticity, and most likely not genuine.
